Schedule a Fertility Evaluation

There are a number of diagnostic tools available to help pinpoint the causes of infertility. Whether you are just beginning evaluations or currently undergoing treatment, we understand how overwhelming the process can be.

Our experienced and compassionate team of specialty trained nurses and doctors are here to provide the guidance and support that you need.


We understand how important your dream of achieving a successful pregnancy is and we are here to help guide you through the fertility evaluation and treatment process.

Infertility affects men and women equally and is defined as the inability to become pregnant, or maintain a pregnancy, after trying to conceive for one year or longer.

If you are under 35: Schedule a fertility evaluation if you have been trying to conceive without success for twelve months or longer.

If you are over 35: Schedule a fertility evaluation if you have been trying to conceive without success for six months or longer.

You do not need a referral or order from a physician to schedule a fertility evaluation.

Initial Visit:
  • Detailed review of medical and fertility histories of each participant
  • An ultrasound exam may be performed to check the condition of the uterus and ovaries
  • Initiation of treatment plans or further evaluation
Subsequent Visits May Include:
  • Physical Exam
  • Blood / hormone testing
  • Follow up ultrasound exams
  • Hysterosalpingogram or Sonohysterogram - a solution will be placed into the uterine cavity and an x-ray or ultrasound completed to determine if the uterus and fallopian tubes are functioning properly
  • Physical examination by a urologist specializing in male fertility
  • Blood / hormone testing
  • Semen Analysis - A semen sample is collected and analyzed for sperm count, movement, and structure

Many insurance companies cover healthcare diagnostic assessments, including fertility evaluations. If your insurance provider does not cover fertility services, we offer affordable evaluations and have programs available to accommodate your needs.
